Information and Consent

I hereby declare that I will not hold Banwell Community Church summer camp, nor any member of it's organization responsible for any accident or injury incurred while participating in Banwell Community Church summer camp activities.

In the event of an emergency, and the contact persons listed above cannot be reached, I authorize the Banwell Community Church summer camp staff to act as an agent for myself to consent to treatment and hospital care advised and supervised by a physician.

I give permission to Banwell Community Church summer camp to utilize any photos taken during camp for photo collages, updates shared with Camp parents, and future camp promotions only.

I have read this form and accept the conditions of registration. I give my child (as listed above) to participate in the activities of the Banwell Community Church summer camp, including the various sports, games, crafts, & other program events under the supervision of Banwell Community Church summer camp staff.
